I. What Is Sorbitol Sweeter?

The chemical name for sorbitol is hexanediol; it is a six-carbon sugar alcohol belonging to the polyol family. In 1872, a French chemist first isolated it from the sap of the rowan tree (Sorbus aucuparia), which is how it earned its name, evoking the fresh scent of plants.   • Powerful Humectant: Sorbitol molecules effectively bind moisture from the environment. This hygroscopic property prevents fillings from drying out in pastries and bread, prevents blooming in chocolate coatings, and serves as a moisturizing and thickening agent in cosmetics, toothpaste, and facial cleansers.
  • Medical Laxative: Sorbitol’s well-known “side effect”—mild laxative action—is, in fact, its intended clinical use. Oral sorbitol solutions can serve as osmotic laxatives, and the enema solutions made from it are widely known. They work by increasing osmotic pressure in the intestines and stimulating water secretion to soften stool. The common belief that “prunes relieve constipation” is based on the combined action of the natural sorbitol and fiber found in prunes.

IV. The Sweet Trap of Sorbitol Sweetener

  • Its water-absorbing properties are both its strength and its weakness. Sorbitol’s laxative effect is precisely the root cause of its most noticeable side effect. The human small intestine lacks the specific enzymes needed to break down sorbitol, so it can only be absorbed slowly and in small amounts; approximately 50% to 70% reaches the large intestine unchanged. There, the gut microbiota ferments it as if it were a feast, producing hydrogen, methane, and carbon dioxide. At the same time, sorbitol itself increases osmotic pressure in the intestinal lumen, acting like a powerful magnet that traps water and prevents its reabsorption. Under this triple assault, mild symptoms include bloating, bowel sounds, and excessive flatulence, while severe cases can lead directly to osmotic diarrhea.
  • There is relatively clear data on this tipping point: a single intake of 10–20 grams of sorbitol in adults can cause noticeable abdominal symptoms, and daily intake exceeding 50 grams puts almost everyone at risk. A 50-gram packet of sugar-free candy, if it uses sorbitol as the primary sweetener, easily exceeds the threshold for diarrhea. For people with irritable bowel syndrome (IBS), sorbitol falls under the “P” (Polyols) category of high-FODMAP foods and is temporarily excluded from a strict low-FODMAP diet to restore intestinal calm.

  • Those with irritable bowel syndrome, as well as anyone preparing for a date, exam, or presentation, should be especially cautious. If you’re extremely sensitive to diarrhea, opt for sugar-free products primarily made with erythritol or stevioside—they’re far less likely to cause digestive upset.
